EMS Reports and Billing

To request an EMS report from the Baltimore City Fire Department (“BCFD”), we will need to collect the following from you:

  1. Request in writing;
  2. Completed and executed HIPAA form signed by the patient or other authorized representative; and
  3. A check in the amount of $25.00 made out to the ‘Director of Finance.’ **
        ** Please note that if you are requesting your own records, no fee is required.

All of these items should be mailed to:

If you are not able to get a signed HIPAA from the patient, you may serve BCFD with a subpoena compliant Maryland Confidentiality of Medical Records Act, § 4-301 et seq. of the Health General Article of the Maryland Code.
